College Readiness Legal Kit (Texas)

What this kit is. Fillable templates and worksheets for a student heading to college in Texas. It includes the official State of Texas medical power of attorney and directive to physicians (both version 05-2024), unchanged. Sign the official forms, not the worksheets that accompany them. The school’s own FERPA release and health forms come first. A person under 18 generally cannot validly sign the adult documents in this kit; the kit includes a consent form for a student who is still a minor. Check the issuing agency’s website for newer versions before you sign.
